TERMS & CONDITIONS

1. Binding Agreement

These Terms & Conditions (“Terms”) constitute a legally binding agreement between you (“User,” “Patient,” or “you”) and Revana Health LLC (“Revana,” “Company,” “we,” or “us”) governing your access to and use of the Revana website, applications, communications, and related technology platform (collectively, the “Platform”). By accessing or using the Platform, you:

- Acknowledge you have read and understand these Terms

- Agree to be legally bound

- Agree to mandatory arbitration and waiver of certain rights

If you do not agree, do not use the Platform.

2. Non-Medical Role of Revana Health LLC

Revana Health LLC is a non-clinical administrative services organization and technology platform.

Revana:

- Does NOT provide medical care

- Does NOT practice medicine

- Does NOT diagnose or treat medical conditions

- Does NOT prescribe medications

- Does NOT supervise providers

- Does NOT establish standards of clinical care

- Does NOT manufacture, compound, or dispense medications

Revana’s role is strictly limited to providing administrative, marketing, operational, and technology support services.

3. Independent Medical Providers & Arora Health, LLC

Medical services available through the Platform are provided exclusively by independent licensed healthcare providers contracted through Arora Health, LLC and its affiliated provider network (“Medical Group”).

Revana Health LLC is partnered with Arora Health, LLC for the purpose of facilitatingpatient access to telehealth services.

All medical providers:

- Are independently licensed professionals

- Exercise independent medical judgment

- Are solely responsible for diagnosis, treatment decisions, and prescribing

- Are not employees of Revana Health LLC Revana does not:

- Direct treatment protocols

- Influence prescribing decisions

- Control provider clinical judgment

- Guarantee issuance of prescriptions

Any provider–patient relationship exists solely between you and the treating provider affiliated with Arora Health, LLC — not with Revana.

4. Technology Platform Disclosure (Smarter EMR LLC dba WIZLO)

The Platform infrastructure and electronic medical record functionality are powered by Smarter EMR LLC dba WIZLO (“WIZLO”), a third-party technology vendor.

WIZLO:

- Provides software infrastructure

- Hosts medical record systems

- Facilitates telehealth communication tools

- Does NOT provide medical care

- Does NOT make clinical decisions

- Does NOT supervise providers

- Does NOT dispense medication

Revana does not control WIZLO’s internal infrastructure, security systems, uptime performance, or third-party integrations. You acknowledge that use of the Platform involves third-party technology systems outside Revana’s direct control.

5. Assumption of Risk

By using the Platform and electing telehealth services, you acknowledge and accept:

- Telehealth has inherent limitations

- Treatment outcomes are not guaranteed

- Medication side effects may occur- Compounded medications may not be FDA-approved formulations

- Shipping delays may occur

- Lab errors may occur

- Technology interruptions may occur

You voluntarily assume all risks associated with medical treatment obtained through independent providers affiliated with Arora Health, LLC.

6. No Emergency Services

The Platform does not provide emergency medical services.

Revana:

- Does not monitor patient conditions

- Does not provide real-time medical oversight

- Is not responsible for delayed responses

In an emergency, call 911 immediately.

7. Prescription & Pharmacy Disclaimer

If a licensed provider affiliated with Arora Health, LLC determines that a prescription is clinically appropriate, prescriptions may be transmitted to one of Revana’s partner

pharmacies.

Partner pharmacies may change at any time without prior notice based on:

- Medication availability

- Ingredient supply constraints

- State licensure requirements

- Shipping logistics

- Regulatory considerations

- Operational capacity

Each partner pharmacy operates as an independent, licensed pharmacy and is solely responsible for:

- Medication compounding and formulation

- Ingredient sourcing

- Quality control

- Sterility and potency standards

- Labeling accuracy

- Dispensing practices

- Storage and shipping

- Pharmacy regulatory compliance

Revana Health LLC:

- Does NOT manufacture, compound, or dispense medication

- Does NOT control pharmacy operations

- Does NOT inspect or test compounded medications

- Does NOT guarantee medication potency, sterility, or efficacy

- Does NOT guarantee shipping timelines

All pharmacy-related inquiries, adverse events, shipping issues, or product quality concerns must be directed to the dispensing pharmacy. To the fullest extent permitted by law, Revana disclaims liability for any act, omission, error, or negligence of any dispensing pharmacy.

8. No Guarantee of Results

Revana makes no representation regarding:

- Weight loss results- Hormone optimization outcomes

- Longevity benefits

- Symptom improvement

- Medication effectiveness

Individual results vary significantly.

9. Platform & Technology Disclaimer

The Platform is provided “as is” and “as available.”

Revana disclaims liability for:

- Platform downtime

- Data transmission errors

- Cybersecurity incidents beyond reasonable control

- Third-party software failures

- Device incompatibility

10. Limitation of Liability

To the fullest extent permitted by law:

Revana Health LLC shall NOT be liable for:

- Medical malpractice

- Prescription decisions

- Adverse drug reactions

- Pharmacy errors

- Laboratory inaccuracies

- Technology outages

- Indirect or consequential damages

- Lost income or business losses

Total aggregate liability of Revana shall not exceed the greater of:

- $500, or

- The total administrative fees paid to Revana in the prior three (3) months This limitation applies under any legal theory.

11. Indemnification

You agree to indemnify and hold harmless:

- Revana Health LLC

- Arora Health, LLC- Affiliated providers

- Smarter EMR LLC dba WIZLO

- Partner pharmacies

- Officers, directors, contractors, and agents

From claims arising out of:

- Misuse of medication

- Inaccurate medical disclosures

- Violation of these Terms

- Failure to follow provider instructions

12. Arbitration & Class Action Waiver

Any dispute arising out of or relating to the Platform shall be resolved exclusively through binding arbitration.

You waive:

- The right to a jury trial

- The right to participate in a class action

- The right to consolidated claims

Arbitration shall be conducted on an individual basis only. This provision survives termination.

13. Governing Law

These Terms are governed by the laws of the State of Florida

14. Severability & Survival

If any provision is unenforceable, the remaining provisions remain in full force and effect. The following provisions survive termination:
